Application for certificate of insurance or other financial security in respect of shipowner liability for the of removal wrecks, etc.
Når ansøgningen indsendes til Søfartsstyrelsen, skal der vedhæftes en erklæring fra forsikringsgiveren eller garanten om, at der for skibet er tegnet forsikring eller stillet anden garanti til dækning af ansvar efter vragfjernelseskonventionen og inden for ansvarsgrænsen i sølovens § 175. I erklæringen skal forsikringsgiveren eller garantistilleren desuden bekræfte, at forsikringen eller garantien dækker det nævnte ansvar. I tilfælde af garanti skal det i erklæringen oplyses, hvori denne består. Erklæringen skal yderligere indeholde ovennævnte oplysninger om skibet.
En forsikringserklæring (Blue Card) kan accepteres i elektronisk form, hvis den er udstedt af en forsikringsgiver, som er medlem af the International Group of P&I Clubs.

An application forwarded to the Danish Maritime Authority must enclose a declaration from the insurer or guarantor stating that insurance has been taken out or a guarantee has been provided to cover liability in respect of the Nairobi International Convention on the Removal of Wrecks, 2007 and within the limit stipulated in section 175 of the Danish merchant shipping act. In the declaration the insurer or guarantor must also confirm that the insurance or guarantee covers the said liability. Furthermore, the declaration must include the above information about the ship.
An electronic copy of an insurance declaration (Blue Card) is acceptable if the insurer is a member of the International Group of P&I Clubs.
